    As a Clinical Nurse Liaison RN you will serve as the initial contact with discharge planners, families, physicians or responsible part to initiate tour and admission to facility.

    You will also manage the discharge process and patient follow-up once the patient returns home.


    In addition, you will establish a strong sales presence with target audiences, including but not limited to: Acute Care providers, physicians and others as identified within the service area to promote awareness of the services for which the community provides.


    Qualifications:

    • Graduate of an accredited school of nursing (RN) with current state license
    • 3-5 years healthcare experience
    • Case management experience preferred
