Phoenix Hospice In-patient Case Managers provide compassionate care to clients and their families dealing with a terminal diagnosis in the hospital setting.

Hospice is a multidisciplinary approach that encompasses the physical, but also the spiritual and psychosocial needs of the patient and their family.

The ideal candidate for this position is an RN with strong clinical skills in clinical assessment, diagnostic interpretation, education, organization and is able to multitask in what can be a fast-paced environment.

Case Managers work alongside the hospitals interdisciplinary team as well as the hospice team to develop and implement a plan of care that is patient/family centered in the line with the hospice philosophy.

• Carrying out physician orders
• Informing the physician of changes in the patient's condition and needs
• Communicating with medical professionals, the hospice team and others
• Maintaining accurate, up-to-date records
• Providing services in accordance with agency policies and regulations that define scope of practice
• Participating in in-service programs
• Demonstrating strong communication and customer service skills

• Kansas or Compact Nursing License
• Hospice experience preferred
• Strong organizational and self-management skills
• Strong communication skills
• Strong and compassionate customer service skills
• Valid driver's license
