    Social Services Coordinator

    We are seeking an experienced professional to fill a Full Time, Days position.

    Job Summary:

    • Demonstrates leadership skills by planning and instituting new ideas and programs that align with the facility's overall program.
    • Receives inquiries from prospective residents and maintains records of interest.
    • Interviews patients or family members when application for admission is made.
    • Explains the cost of care involved in the appropriate level of care determined by the admitting physician.
    • Provides necessary forms to request medical and social information.
    • Acts as liaison between families, hospital discharge planners, social service agencies, and physicians.
    • Notifies the family of approval for admission and room assignment.
    • Notifies all relevant departments of admission.
    • Greets patient and family at the door and assists with settling in.
    • Completes the MDS form.
    • Chairs the Admission Conference/MDS meeting for new admissions.
    • Holds patient-family conferences as needed.
    • Attends required meetings.
    • Assists with discharge placement and provides community resource lists.
    • Investigates and reports resident complaints to the Department Director.
    • Reviews and implements change of room requests.
    • Refers residents or families to external resources when needed.
    • Collaborates with Unit Directors/RN Supervisors, administrative nursing, and other Social Workers.
    • Writes correspondence following established procedures.
    • Monitors MDS completion and quality assurance.
    • Ensures accurate documentation and regular visits with residents.
    • Participates in Family Council and Quality Assurance committee.
    • Conducts regular meetings with Social Workers.
    • Maintains a professional atmosphere and supports Facility objectives.
    • Assists with annual staff services.
    • Fulfills emergency procedures during fires or drills.

    Job Requirements:

    • High school diploma required; one year of experience in healthcare or long-term care or equivalent combination of education and experience.
    • Ability to work effectively with others and problem-solve under pressure.
    • Displays neat appearance, good personal hygiene, and business attire.
    • Able to read, know, and follow policies.
    • Familiarity with local, state, and federal regulations regarding social needs of patients.
    • Valid driver's license required for occasional transportation duties.
