    Job Title: Director of Nursing at Arizona's Choice Home Health Agency



    Salary:

    $85,000 - $95,000 per year depending on experience, plus annual bonus.

    *Annual bonus target of 10% of annual salary

    Location: Surprise, AZ

    Type: Full-Time

    Company Overview: Arizona's Choice Home Health Agency (ACHHA) is seeking a dedicated Director of Nursing to ensure clinical compliance and maintain licensure standards for the company.

    Essential Functions:

    • Requires minimal direct supervision
    • Implement quality improvement changes for patient care enhancement
    • Oversee state licensure and compliance with regulations
    • Set professional and ethical standards for home care personnel
    • Delegate tasks effectively and ensure high standards of personnel education
    • Collaborate with Quality Assurance Program RN for patient record compliance
    • Lead quality initiatives and ensure regulatory survey readiness
    • Provide clinical input to support company decisions
    • Be the clinical point person for home care services
    • Participate in public relations and community activities
    • Demonstrate competence in home care evaluation and interventions
    • Ensure accurate documentation and adherence to corporate policies
    • Provide direct patient care and nursing services as required
    • Coordinate services with other healthcare providers
    • Perform chart audits and ensure compliance
    • Maintain high clinical compliance and patient care utilization
    • Carry out other duties as assigned

    Requirements:

    • Maintain current state licensure and liability insurance

    Education/Training:

    Required:

    • Associate's degree in a related field
    • Current RN license
    • Basic Cardiac Life Support certification

    Preferred:

    • Minimum 4 years of RN experience
    • Minimum 2 years of nurse management experience
    • Home health experience preferred

    Licensure/Certification:

    • Valid Driver's License
    • Current CPR Certification
    • Valid RN license

    Clearances:

    • Criminal background check
    • Driving Record check
    • OIG Exclusion List check
    • Sex Offender Registry check
