Job summary
This is a Direct Hire Solicitation for a Nurse position located in Family Medicine at the Madigan Army Medical Center at JBLM, WA. The nurse will serve as a liaison between healthcare providers and members of the primary care team to identify beneficiaries with short- and long-term health care needs. The position requires standardizing Disease Management across the patient care continuum by monitoring disease management of patients in accordance with clinical practice guidelines.Qualifications
The qualifications include a graduate or higher-level degree from an accredited professional nursing educational program and active registration as a professional nurse from a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.Job description
