Job Summary
The Annual Wellness Visit (AWV) Nurse works collaboratively with the primary care provider, care coordination team, and all members of the healthcare team.Principal Responsibilities
- Will participate and maintain a culture within The Corvallis Clinic that is consistent with the content outlined in the Service and Behavioral Standards Handbook.
- Engage and empower patients to participate in the preventative care approach.
