    Description

    Infection Prevention LVN needed

    Provides nursing care within scope of practice to ensure patient's needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

    QUALIFICATIONS:

    · 2-3 years of clinical experience in long-term care preferred.

    · Infection Control Certification Required or the ability to obtain within 30 days of hire.

    · Must have training in infection prevention and control in accordance with federal requirements.

    · Maintain line listing for staff and residents, maintain records of vaccinations for staff and residents, maintain monthly surveillance reports, lead staff, residents, and visitors in safe IP practices, ensure annual fit testing for N95s are completed, etc.

    · Participates in required meetings, inservices, admission reviews, input/edit orders as prescribed, labs/diagnostics, etc. to coordinate compliance with infection control policies and practices.

    · Directs and coordinates compliance with the Facility's Antibiotic Stewardship Program and Infection Control Policies and Procedures.

    · Collaborate with Nursing, SSD, & Admissions Staff to ensure new and modified room assignments comply with infection standards for reducing pathogen transmission.

    · Must be willing to seek out new methods and principles and be willing to incorporate them into existing training practices, new hire training and annual training.

    · Utilizes epidemiologic principles to conduct surveillance activities and investigations.

    · Must possess the ability to engage tactfully with regulators, facility personnel, residents, visitors, and the general public.

    · Point click care charting experience

    · Current LVN/LPN license in practicing state.

    · Current BLS/CPR certification or willingness to acquire one.

    RESPONSIBILITIES:

    · Follows established procedure for charting and incident reporting.

    · Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.

    · Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.

    · Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.

    · Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.

    · Makes rounds with physicians when necessary.
