Job Overview:Shriners Children's Ohio has an opportunity for a part-time Social Worker for our Care Management. Join a team of professionals committed to improving children's lives. Our top healthcare professionals excel in their practices and provide compassionate, family-centered care to children who need it most. Hours: The hours for this part-time position is 20 hours per week and the preferred schedule is 2.5 days per week.
Responsibilities:The Social Worker is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.
Education
Masters in Social Work required
Certification
Ohio Social Work license
Case Management Certification (CMC, ACM or ANCC) required or the ability to attain certification within 2 years of hire or within 2 years of eligibility
Experience / Skills
Three years of social work experience required; 5-7 years preferred
Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educational needs of the patient/caregivers, required.
* Experience in a Healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigating psychosocial barriers to patient/family adherence, in a Pediatric setting is preferred.
Bilingual in English/Spanish preferred
