    Summary

    Position Summary


    The Stroke Program Coordinator is responsible for oversight and management of the Southern Maine Health Care (SMHC) Stroke Program, including clinical treatment protocols, performance improvement and the facilitation of the interface between SMHC and outside programs.

    This position will coordinate multidisciplinary care related to acute stroke care delivery and enhance research in the clinical setting. Additional duties include monitoring quality of care issues and managing stroke care pathways including patient education. This position also acts as the clinical liaison between physicians, patient care leaders, and administrative staff.

    The Stroke Program Coordinator will oversee tracking of all programmatic data and serve as an educational resource for the Stroke Program to providers, staff and patients, and community hospitals and physicians, with the goal of attaining Joint Commission Primary Stroke Center Certification.

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

    Education

    :

    Bachelor's Degree in nursing and currently licensed in Maine. Master's degree preferred.

    License/Certifications

    :

    Certification in NIH Stroke Scale (NIHSS) and modified Rankin Scale (mRS) or willingness to pursue.

    Experience

    :


    Five years of experience in the processes of care for diagnostic evaluation and treatment of stroke and other vascular disease.

    5+ years' experience leading a clinical program or multidisciplinary work group in a health care related setting preferred.

    Experience in serving as a liaison between health care institutions and community based organizations.

    Strong written and oral communication skills.

    Ability to identify and build relationships with local resources.

    Experience with implementation of education plans.

    Ability to supervise and hold accountable data abstraction staff.
