    We are professional, supportive, autonomous and our goal is to provide exceptional and knowledgeable nurses to administer outstanding home care for all patients with a wide variety of needs.

    Our work environment includes:
    • Relaxed atmosphere Safe work environment Non Corporate setting. We are looking for Licensed Practical Nurses who have a background in the home health industry to join our team.
    • The successful candidate will have the opportunity to create a schedule for the day and interact with patients, physicians, and fellow colleagues. We are seeking a dedicated, caring individual with a strong commitment to helping others and who will contribute positively to our team.
    • The successful Licensed Practical Nurse candidates will possess excellent communication skills, interpersonal skills, and organizational skills all while maintaining professionalism in their work.
    • The candidate will rotate on-call weekends/backup (1 weekend per month) as well as rotating weeknight on-call (usually 3 days a month) and holidays. We also include $150 for being on weekend call plus per visit rate with the mileage, and $100 for backup on call plus per visit rate with mileage. Faith Home Care is also offering a $2,000 sign on bonus to be paid $500 every 90 days.

    Responsibilities:
    • Administer medications and treatments in accordance with physician's orders, governmental regulations, and professional standards.
    • Monitor the condition of patients to detect any changes that may require intervention by medical personnel. May provide care and treatment to patients with intellectual and physical disabilities, using knowledge acquired through educational training.
    • Assist in monitoring patients' for medication interactions, side effects, as well as educating patients and caregivers on uses, side effects, and interactions of medications.
    • Complete skilled nursing tasks including injection administration, dressing changes, foley and suprapubic cath changes, venipuncture and full patient assessments.
