The Clinical Pharmacy Specialist plays a vital role in contributing to the prevention and cure of cancer by providing safe, evidence-based, cost-effective, and patient-centered medication management.Responsibilities
- Provide oversight for the safe and appropriate use of medication prescribing dispensing administering and monitoring Deliver clinical pharmaceutical care to optimize drug therapy for cancer patients through recommendation implementation monitoring modification of individualized treatment plans in collaboration with the healthcare team Perform medication order review clarification verification assessing for completeness appropriateness Supervise accurate hazardous non-hazardous drug preparation distribution per policies procedures Process double-check chemotherapy orders as necessary Develop interpret provide medication-related information education pharmacists providers nurses other healthcare personnel Coordinate provide medication therapy education counseling patients caregivers Precept pharmacy students residents others training Participate clinical research projects Provide leadership developing implementing maintaining medication-related guidelines policies standards Manage mediation use process improve workflow quality safety patients staff Oversee cost-effective medications ensure appropriate utilization resources reduce financial risk
