    Housing Stabilization Services Worker

    Job Summary:

    We are seeking a dedicated and compassionate Housing Stabilization Services Worker to provide critical support to individuals with disabilities, helping them achieve stable housing and independence in their communities.

    About the Role:

    This is a unique opportunity to make a meaningful difference in the lives of vulnerable populations. As a Housing Stabilization Services Worker, you will work closely with clients to develop personalized plans, providing assistance with goal setting, skill building, client advocacy, and connecting them with essential community resources.

    Key Responsibilities:

    • Provide community-based Housing Stabilization Services (transition and sustaining) to eligible adult clients with disabilities.
    • Maintain a caseload of at least 10 clients, meeting productivity guidelines, and proactively seek referrals as needed.
    • Develop and implement person-centered housing plans, ensuring services are culturally sensitive and tailored to each client's needs.
    • Assist clients with accessing benefits, researching housing options, and navigating lease agreements.
    • Support clients in maintaining stable housing through prevention and early identification of potential risks.
    • Collaborate with clients, families, and other stakeholders to ensure seamless service delivery.
    • Document all interactions, progress, and outcomes in accordance with Department of Human Services guidelines.

    Requirements:

    • Bachelor's degree in social sciences or related field (e.g., Social Work, Psychology, Human Services, Chemical Dependency, Sociology).
    • OR at least one year of experience working in a community setting, providing similar services.
    • Valid Minnesota driver's license, reliable transportation, and valid insurance.
    • Excellent communication, observation, and crisis management skills.
    • Ability to handle stress, work independently, and maintain confidentiality.

    Work Environment:

    This role requires frequent travel in your personal vehicle to clients' homes and community locations. Normal office conditions exist, with occasional moderate noise levels.

    Physical Demands:

    Frequent standing, walking, sitting, bending, twisting, talking, and hearing are required. Occasional lifting up to 50 pounds may be necessary.

    Mental Demands:

    You must be able to handle deadlines, interruptions, and multiple tasks while interacting with various people on complex issues.

    Confidentiality and Background Investigation:

    All employees must uphold confidentiality principles and successfully pass a required background check to qualify for this position.
