- Associate's Degree in Human Services, or related field desirable or experience as an MT, Paramedic, or Certified Nursing Assistant or related medical background also applicable.

Hometown Health Center is hiring a Community Health Worker (CHW). Hometown Health Center is a Federally Qualified Health Center in rural central Maine that provides comprehensive health care including medical, dental, and behavioral health services. The CHW position is responsible for helping patients and their families navigate and access community services and resources. The CHW supports the patient care team through an integrated approach to care management and community outreach, focusing on activities that promote, maintain, and improve the health of patients and their families.
