    This is a full-time teleworking position with the expectation of visiting clients in the Central Maryland region and, on occasion, working from our office location in Millersville, MD.

    • RN, LMSW, LCSW, LCSW-C license required, RN license preferred.
    • Bachelor's degree in a health-related field required. Master's degree, preferred.
    • CCM or ACM certification required.
    • At least five years of robust case management experience with medical-based clients.
    • Excellent analytical, problem solving and interpersonal communication skills with a strong attention to detail.
    What You Will Do
    This position is responsible for providing care management services to recipients of trusts and other similar private funding arrangements. This position supports the global needs of private care management clients served by the Medical Legal Services division and enhances its operations and development. It includes the coordination of medical care, rehabilitation therapy, community inclusion, residential services and other needs identified in the client's care plan to maintain their optimal health and well-being in the community. You will work in collaboration with other members of the Medical Legal division including research and administrative support.
    This position also includes the commitment of the candidate to obtain certification as a life care planner within 15 months of beginning employment with The Center. Once certified, you are responsible for providing life care plans and related litigation support services as requested by referring parties. As a certified Life Care Planner, you will provide subject-specific expertise and training to all division members, contributing to the high standards of The Center and applicable Standards of Practice in life care planning and case management. This position embodies the highest level of care coordination knowledge and practice with the life care planner demonstrating ease and confidence in the presentation of the client's needs through the life care plan and/or in educating the referring party relative to the needs of the client throughout the anticipated lifespan.
    As a Life Care Planner, you will:
    • Complete care management assessments and the establishment of care management goals with the client and any personal support team.
    • Work in collaboration with the client, their family members, their trustee, physicians, therapists, school, community-based programs, other care managers and any other team members and/or individuals involved in the client's day-to-day care to achieve their goals and those identified in the care plan.
    • Utilize phone contacts, onsite visits, and participation in multi-disciplinary team meetings to support the achievement of care plan goals.
    • Locate and coordinate appropriate goods, services, and resources to meet the client's needs and goals, both short- and long-term including rehabilitation, residential and transportation needs.
    • Document all interventions with and on behalf of the client in identified software programs.
    • Demonstrate an ability to effectively complete all phases of the life care planning process including medical records reviews, interviews of the evaluee, research, report preparation and calculations.
    • Develop life care plans consistent with the standards of practice in life care planning and/or nurse life care planning and with the needs of the evaluee/individual as represented by treating and/or expert providers or otherwise reflected in established health and rehabilitation standard of practice and clinical practice guidelines.
    • Maintain a current understanding of the litigation process and relevant standards of practice, changes in rulings and rules of evidence as they impact the life care plan and planner and assure that their life care planning practices and products incorporate this understanding.
    • Effectively collaborates with referring attorneys and others including treating and expert medical and rehabilitation providers to obtain clear information/recommendations to be reflected in the life care plan.
    • Demonstrate an ability to effectively communicate the needs of the evaluee and defend the opinions in the life care plan through testimony, both deposition and court testimony.
    • Demonstrate a consistent ability to provide case management services to a variety of individuals who are the recipients of third-party payment structures.
    • Establish and monitor your own workflow and productivity to assure the completion of tasks and assignments within anticipated timeframes and deadlines as established by the referring party and the division's goals for development and fiscal soundness.
    • Represent The Center's Medical Legal Services division at relevant professional meetings and consortia, providing information and resources relative to life care planning and community-based care coordination services.
