    The Licensed Practical Nurse (LPN) position provides direct patient care, community outreach and support for prescribers in behavioral health clinics. They participate in the planning, implementation, and evaluation of nursing care, based on the objectives, standards, and policies of practical nursing. The Licensed Practical Nurse (LPN) is a positive team player that promotes a culture of inclusion and participation.

    Required Education and Experience

    • Active LPN licensure with a minimum Associates degree (with no restrictions) from the Kentucky Board of Nursing
    • Must have a valid driver's license, reliable operating transportation, and proof of current auto insurance
    Preferred Education and Experience
    • Experience in office-based setting
    • Experience with psychiatric nursing
    Essential Functions
    • Request KASPER report on behalf of prescriber
    • Instructs and counsels patients regarding tests and collection of specimens; assures proper handling and transporting of laboratory specimens
    • Assist patients in applying for PAP, CMSP and other medication assistance programs
    • Assist prescribers in obtaining prior authorizations for medications/services
    • Assist prescribers with documentation of treatment planning
    • Schedule and provide IM injections in office as ordered by prescriber
    • Accompany clinicians in the community providing IM injections as ordered by prescriber
    • Take phone calls for prescriber regarding medication issues and forward them as appropriate
    • Perform vitals and record them for the prescriber to review if needed
    • Assist in supports to provide telehealth services if needed
    • Maintain samples and medication closet
