    Referral Coordinator RN - IP Rehab Unit at Providence Queen of the Valley Medical Center in Napa, CA. This position is Part-Time and will work 8-hour Day shifts.

    Under the general supervision/direction of the Acute Rehabilitation Nurse Manager, and in collaboration with the treatment team and external entities, the Rehabilitation Admissions Nurse shall monitor, coordinate and facilitate the interdisciplinary plan of care, effective clinical resource management, discharge planning, and continuation of post acute care needs of patients assigned.

    Required Qualifications:
    California Registered Nurse License upon hire.
    National Provider BLS - American Heart Association upon hire.
    2 years - Rehabilitation Nursing.

    Preferred Qualifications:
    Certified Rehab Registered Nurse (CRRN) upon hire.
    Bachelor's Degree - Nursing.

    The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.

    These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
