Responsibilities

  • Conduct comprehensive needs assessments and develop individualize care plans
  • Maintain accurate documentation What you'll bring: Your passion for helping others and desire to make a difference Adaptability to be able to change as the needs of consumers and/or programs change At least one year of experience in the human services field Bachelor's degree in Social Work or related field highly preferred; Current licensure if appropriate What you'll get: Ability to work with an enthusiastic team of like-minded individuals Opportunity to Develop Professionally In Your Chosen Career Great Work/life Balance
    35-hour work week with some flexibility; standard M-F workweek; hybrid Work Environment (home, Office And Home Visits).
