- Conduct comprehensive patient assessments and develop patient-centered care plans in collaboration with physicians and interdisciplinary teams.
- Monitor and evaluate patient progress, adjusting care plans as needed to achieve optimal outcomes.
- Provide skilled nursing care, including wound care, medication management, and patient education, in the home setting.
- Coordinate care with other healthcare professionals and make appropriate referrals as needed.
- Ensure timely and accurate documentation of patient assessments, interventions, and outcomes.
- Educate patients, families, and caregivers on safe care practices and self-management techniques.
- Adhere to scope of practice, agency policies, and regulatory requirements at all times.
- Participate in quality improvement initiatives and professional development activities as required.
- Perform weekend and on-call duties as scheduled.
- Valid RN license in New Hampshire (NH) or Maine (ME)
- Has Graduated from an approved Nursing curriculum by the state and by the designated nursing governing board
- Preferred minimum of 1 year of clinical nursing experience, preferably in home health or a related setting.
- Strong assessment, critical thinking, and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a team.
- Proficiency in electronic medical records/documentation.
- Willingness to work weekends and participate in on-call rotation.
