    *Job Description: As a Home Health Registered Nurse (RN) at Cornerstone VNA, you will play a crucial role in coordinating and directing the care of our home care patients. Working independently and under the general supervision of a Team Manager, you will assess patient needs, develop individualized care plans, and provide skilled nursing care in accordance with clinical best practices. You will also collaborate with interdisciplinary teams, educate patients and caregivers, and ensure compliance with regulatory requirements.

    *Key Responsibilities:
    • Conduct comprehensive patient assessments and develop patient-centered care plans in collaboration with physicians and interdisciplinary teams.
    • Monitor and evaluate patient progress, adjusting care plans as needed to achieve optimal outcomes.
    • Provide skilled nursing care, including wound care, medication management, and patient education, in the home setting.
    • Coordinate care with other healthcare professionals and make appropriate referrals as needed.
    • Ensure timely and accurate documentation of patient assessments, interventions, and outcomes.
    • Educate patients, families, and caregivers on safe care practices and self-management techniques.
    • Adhere to scope of practice, agency policies, and regulatory requirements at all times.
    • Participate in quality improvement initiatives and professional development activities as required.
    • Perform weekend and on-call duties as scheduled.
    *Qualifications:
    • Valid RN license in New Hampshire (NH) or Maine (ME)
    • Has Graduated from an approved Nursing curriculum by the state and by the designated nursing governing board
    • Preferred minimum of 1 year of clinical nursing experience, preferably in home health or a related setting.
    • Strong assessment, critical thinking, and decision-making skills.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and collaboratively within a team.
    • Proficiency in electronic medical records/documentation.
    • Willingness to work weekends and participate in on-call rotation.
