Job summary
This employee will provide case management services to adults with a diagnosis of a severe mental illness that are working with the AOT Program and Mental Health Court.Duties include:
- Provide intensive case management to identified clients.
- Have ongoing contact with community agencies necessary to develop additional resources and coordinate services.
