    As the Community Health Screener, is an entry-level clinical role that supports the Community Engagement Coordinator at community engagement events.

    This role interacts directly with community residents, performs basic health screenings, and provides basic health education under the direction and supervision of the Community Engagement Coordinator.


    JOB CRITERIA AND PERFORMANCE STANDARDS:

    Performs blood pressure, cholesterol, HbA1c and other health screenings under the direction and supervision of the Community Engagement Coordinator.

    Identifies participants with a health risk based on screening results and provides basic, linguistically, and culturally appropriate health education to participants under the supervision of the Community Engagement Coordinator.

    Ensures that all community residents participating in health screening events have completed and signed an informed consent and demographic data collection form and provides assistance to participants as needed.

    Utilizes interpreting equipment (e.g. Pocketalk devices) as needed to effectively communicate with community members. Helps with event set up and break down, loading and unloading supplies from the Mobile Health Unit (MHU).

    Assists the Community Engagement Coordinator with post-event follow up as needed to ensure at-risk participants are able to obtain follow-up care and services.

    Serves as a resource to community members who identify a medical or social need, directing participants in need to relevant medical or community services.

    Serves as a hospital ambassador, demonstrating compassion and respect in call interactions. Maintains patient privacy as required by law and hospital policy. All other duties as assigned.


    Requirements:
    Minimum 2 years of experience as a Nursing Assistant or Trauma Technician, Medical Assistant, Emergency Medical Technician, or similar role Excellent interpersonal and customer service skills BLS Certification

    Preferred:
    Bilingual (Spanish/English) CNA or MA certification # #
