    Lakewood Family Health Center has been providing top-quality patient care and a wide variety of specialties to the communities in and around Lakewood since opening in the summer of 2018.

    Lakewood Family Health Center also offers multiple specialty centers, such as a Center for Family Medicine, Center for LGBTQ+ Care, Emergency Department and more.

    As a Community Outreach Patient Navigator, you will assist our patients by overcoming any barriers for receiving cancer screenings. You will be serving our community and patients by decreasing disparities through navigation and education.


    Your duties will include:
    Building relationships, solving problems, removing barriers to care and supporting patients. Educating people within the community on disease prevention, and necessary health screenings. Coordinating appointments and testing for early detection of medical issues. Assisting with insurance and financial needs, transportation, knowledge and other psychosocial concerns.

Thisrole is hybrid, we anticipate you to mostly be at Lakewood Family Health Center or Fairview Hospital. This position may require additional travel to other locations in Northeast Ohio.

    Responsibilities:


    Coordinates and educates patients within the community on disease prevention and screeningIdentifies and addresses barriers to care, fear, trust, financial, insurance, transportation and psychosocial to ensure appropriate clinical care is coordinated and provided in a timely mannerHelp patients navigate our complex health care systemsCoordinates appointments related to health screenings i.e.

    mammogram results, colonoscopy results and pathology reportsNavigates patients to appropriate health care provider within all Cleveland area facilitiesIdentifies barriers to care and coordinates resolution to including but not limited to:
    Social work supportMedicaid and MedicarePrescription Assistance ProgramsCommunity resources (gathering place & 211)

    Connecting patients to primary care and medical homesSupport coordination and execution of community eventsEducation:
    High school diploma required

    Languages:
    Spanish (orally and written) preferredCertifications:None RequiredCompetencies (Complexity of Work):Knowledge of Electronic Medical Record (EMR) preferredAbility to deal effectively with a variety of people and work in a team environmentExcellent patient service orientation skillsMust possess strong oral, written and organizational skills

    Work Experience:
    Minimum three years related experience in customer relations with diverse patient/culture populations, exhibiting problem solving skills.
    Excellent interpersonal skills with special emphasis on oral and written communication, process improvement, and problem resolution.
    Requires strong communication, analytical, and critical thinking skills.


    Preferred Experience:


    EPIC or other Electronic Medical Record experience terest in advancement of LGBTQ+ healthcare utilization / health equity - strongly recommended.

    Prefer candidate has experience in and/or understanding needs of LGBTQ+ community.
    Prefer candidate looking for upward mobility. Opportunity for advancement and promotion expected in 2-3 years.


    Physical Requirements:
    May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing.
