Responsibilities include the following.+ You will serve as a clinical co-pilot—charting the best trajectory of each patient's care by accommodating various needs, including medical, psycho-social, financial and practical.+ You will join a shared culture of passion, compassion and sophistication that will make your work experience here exhilarating.+ Your curiosity and dream to broaden your scope of experience will grow and thrive in our Magnet-designated, teaching environment, having all the advantages of the most sophisticated resources and brand-new capabilities.+ You will coordinate the care and service of the patient population from admission through discharge.+ You will assess and Plan, implement, monitor, and evaluate options and services to meet an individual's health needs to facilitate a safe and timely discharge to the next appropriate level of care.
Qualifications*Educational Requirements:
BSN preferred.
Associates Degree in Nursing required, CA License requiredLicenses:
Experience and certification in Case Management preferredCase Management certification expected within 2 years of hireBLS CertifiedExperience:
3 years minimum in Acute Care Nursing
