- Respond to calls from Triage, visit patients with changing conditions, and complete weekend admissions.
- Conduct comprehensive assessments of patients and families to determine hospice needs.
- Provide professional nursing care using the nursing process.
- Develop care plans, administer medications, and provide health care instructions.
- Assist patients with daily activities and promote self-sufficiency.
- Ensure a safe environment for patients and coordinate care with the interdisciplinary team.
- Attend meetings, participate in quality assessment teams, and maintain professional development.
- Supervise ancillary personnel and delegate responsibilities when needed.
- Fulfill case assignments and contribute to performance improvement activities.
- Graduate of an accredited nursing school with 1-2 years of acute care experience.
- Current state licensure and CPR certification.
- Strong communication, problem-solving, and nursing skills.
- Physical ability to lift patients and equipment and perform required nursing skills.
- Valid driver's license and insured automobile in good working condition.
- Work closely with patients, impacting their lives and families directly.
