    General Description:

    Manages common acute and chronic health care problems; performs as an interdependent member of the health care team through direct medical care, consultation, collaboration, coordination and referral; practices to his/her maximum potential as in accordance with state and federal laws; functions primarily as a clinician but also has consultative and educational responsibilities; works collaboratively with all care team members supportive of the patient centered medical home model

    Essential Duties & Responsibilities
    • Clinical - Provides health care through health promotion, illness prevention and health restoration
    • Consultation - Provides consultation to patients, their families, peers, physicians, other health care providers and care team members on health maintenance visits, educational programs, individual referrals, peer and nursing staff resources
    • Administrative - Attends QA meetings, patient care and program review meetings as designated by Medical Director. Attends appropriate continuing education sessions to maintain state licensure as a Physician Assistant
    • Educational - Teaching patients and families community health promotion and illness prevention strategies. Mentors and serves as a preceptor to Physician's Assistant students

    Minimum Qualifications:
    • Critical Thinking, delegation skills, preceptor skills
    • Quality Improvement Process
    • Ability to navigate and enter data into an electronic health record
    • Attendance and graduation from an accredited APRN certification program
    • Valid and current state of Florida APRN License
    • DEA certificate
    • Three (3) years of APRN practice experience in a clinic or hospital-based setting
    • Current BLS certification
