    Position: Community Liaison

    Location: Honolulu, HI

    Pay: $45/hr

    Experience: Two (2) years of clinical or related experience (case management, rehabilitation, primary care, chronic disease management, home health care)

    Education: Graduate from an accredited school of Nursing

    Type: Full-time; Direct Hire

    Schedule: Monday – Friday, 8am-5pm

    Kahu Malama Nurses is seeking a Community Liaison to join our growing and dynamic team

    Job Description:

    • Handle assessment screenings and admission of patients to services
    • Identify individuals who meet program criteria and will benefit from services offered
    • Serve as a clinical practitioner, educator, advocate, consultant, facilitator, negotiator, and collaborator
    • Assist with promoting and marketing programs and services
    • Utilize resources available for assessing regulations and the parameters of third-party reimbursement
    • Provide an understanding of the insurance and managed care arenas to help educate others
    • Other duties and responsibilities as assigned

    Position Requirements:

    • Current license as a Registered Nurse in the State of Hawaii
    • Current BLS certification
    • Valid State of Hawaii driver's license
    • Basic computer skills
    • Excellent written and verbal communication skills
    • Strong assessment, analysis, and decision-making skills
    • Effective time management skills
