Provides physical therapy treatment under the direction of a licensed Physical Therapist according to the principles and practices of Physical Therapy.
Accountabilities
* - Demonstrates basic knowledge of diagnoses/conditions seen in physical therapy
* - Interacts with others involved in the patient's care to incorporate an interdisciplinary treatment approach for coordinated quality care
* - Demonstrates awareness and use of proper safety measures regarding the patients, others in the home and self
* - Assesses patient response to treatment and modifies goals and treatments as indicated
* - Provides information regarding follow-up services which may include a referral to other community resources
Actively participates in patient and family education process. 15%
* - Supports and provides educational material to patient/families for support groups as well as community resources
* - Provides education and training to patients, families, and caregivers regarding diagnoses, progress, and long-term functional outcomes
* - Teaches, counsels, and communicates with the patient, family or caregiver educational information related to the patient's diagnosis/condition
Complies with departmental policies for documentation. 15%
* - Documentation is in compliance with chart review audit components
* - Documents all aspects of assessment, planning, treatment and discharge in the computer in a timely manner
* - Follows POC as defined by the primary therapist and documents the patient's progress toward goals
* - Daily treatment records are noted in the computer on a daily basis and are clear and professional in terminology.
* - Appropriate abbreviations and medical terminology is utilized
* - Obtains re-evaluations as required by S. C. Physical Therapy Practice Act
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
