Job summary
The Nurse Aide II functions in an assistive role in the provisions of Client Care Activities through regular assignments or delegated tasks all supervised by the RN.Duties
- Each shift or visit reflects the provision of care rendered in accordance with Physician's orders/Plan of Care/Home Health Aide Assignment Sheet.
- Document all tasks completed in a timely complete and accurate manner Tasks may include:
Checking/documenting patients pulse temperature and respiration Assists patients in simple prescribed exercises and range of motion Maintains records to report and discuss observations with a supervisor or case manager Provides patients with help moving in and out of beds baths wheelchairs or automobiles and with dressing ang grooming Changes bed linens washing and ironing patients laundry cleaning patients quarters Performs a variety of duties as requested by client running errands preparing meals etc Facilitating communication with supervisory RN by requesting clarification affirming task expectations Functions as a team member demonstrating collaborations with responsibility to patient physician healthcare team Shifts visits are according to orders on Plan o fCare/Home Health Aide Assignment sheet Observations significant changes patient status communicated RN Case Manager Attendance Multi-Disciplinary Team Meetings is recommended documented via sign-in sheets Communicates RN Case Manager/Director Clinical Services each patient needed Asks questions seeks clarification Informs nurse if UAP has never performed task infrequently Requests additional training guidance needed Affirms understanding acceptance delegation Complies communication between nurse UAP Reports care results nurse timely manner Complies emergency action plans Participation Agency other mandated activities requested Agency Director In-Services Supports executes mission ethics goals company effectively All clinical documentation completed accurately submitted timely per agency policy/guidelines Represents self positive professional manner company community Adheres dress code clean neat professional appearance Reports time scheduled complete work designated time Adhere company policies procedures outlines Employee Handbook Employees Agreement communicated executive team
