Overview
Gifford has an opportunity for a Post-Acute Care Clinic Nurse Practitioner to join our well established, successful Primary Care Division.
Discharge from an inpatient or ED stay can be confusing for patients and their families and this position helps provide review, education and direction to patients to ensure their understanding of "next steps" in their care plan.
The Clinician is expected to speak with patients about their illness, educating their family members, and partnering with other Clinicians to provide high quality, evidence-based care.
This is a unique opportunity to work in a spectacular setting at a rural, financially stable, non-profit hospital with a progressive philosophy supportive administrative team and advanced technology.
