CGRC's SHAIP (Supportive Housing Aging in Place) program is hiring for a part time Licensed Practical Nurse .

SHAIP is a supportive housing service in Delaware County designed to meet the needs of the older adult (ages 55 and older) priority populations.

Residents may include, but are not limited to, medically fragile, individuals with mental illness, co-occurring drug and alcohol disorders, and intellectual disabilities.

The residents of SHAIP do not meet criteria for nursing homes, but they do experience significant cognitive, medical, and mental health challenges and need living support.

Follow through with medical treatment;
Check and document weights & vitals;
Blood sugar testing for residents diagnosed with Diabetes;
Communications with specialists & hospital personnel, as needed;
Complete annual mandatory training requirements;
Participate in team meetings and training activities as required;
Perform other medical-related duties as assigned or requested to support the SHAIP clients.
