Responsibilities

Evaluates assigned patients establishes appropriate treatment goals treatment programs.
Provides ongoing evaluation patient status goals modifying treatment program timely progression.
Develops comprehensive goals patient treatment maximize patient outcomes return community activities.
Captures data evaluates reports results of interventions appropriately documents accurately completes required paperwork records meets established deadlines provides accurate complete documentation according company policies procedures guidelines quality standards regulations federal state local laws rules credentialing accreditation bodies regulatory agencies.

Coordinates with other clinical therapeutic services well available community resources attends necessary meetings facilitates coordination.
