- Conduct comprehensive assessments of clients' overall healthcare needs, considering familial and community aspects.
- Evaluate social and emotional factors influencing patients' conditions, treatment response, and adaptation to care.
- Collaborate with clients, families, and healthcare team to develop and implement care plans aligned with physician directives and organizational guidelines.
- Execute care plan activities to promote patient and family independence towards self-sufficiency.
- Provide support for staff learning initiatives and participate in case discussions when necessary.
- Master's or Doctoral degree from an accredited social work school recognized by the council on social work education.
- Active Maine Board of Social Work Licensure: LCSW, LCSW, LMSW-CC, or LSW mandatory.
- Valid CPR certification.
- Over one year of experience as a master's level social worker in a healthcare environment.
- Possession of a valid Driver's License and access to a reliable insured vehicle.
