    We are currently looking for Patient Care Coordinators to join our Team.

    1 position to work M-F 08:30am-5:00pm

    1 position to work M-F 08:30am-5:00pm - Bilingual English and Spanish

    1 position to work M-F 07:00am-03:30pm

    As a Moments Hospice as a Patient Care Coordinator, you will be responsible for answering phones. Creating positive interactions as the first line of interaction with customers and referral sources.

    Must be able to work in a fast paced environment and Have excellent communication skills.


    Job Description:
    Answer phone calls and incoming patient messages.

    Direct phone calls.

    Responsible for medical records in Forcura (uploading into HCHB).

    Conduct or assist with special projects as requested

    Must be able to work in a fast paced environment and have excellent communication skills.


    Required skills and qualifications:
    High school diploma or GED; Associate or Bachelor's degree preferred

    Two years of office experience, preferably in a hospice agency setting

    Proficiency with computers and software applications, including Microsoft Office

    EMR software experience highly preferred

    Hospice experience preferred

    HCHB experience preferred

    Forcura experience preferred
