Job summary
The Care Coordinator Concierge will provide high-touch service to patients of Loyola Medicine. Partnering with the Concierge Medical Director and other internal stakeholders, the Care Coordinator will serve as primary point of contact to coordinate scheduling of appointments navigate emergency and inpatient requests enlist guidance of medical director when escalation is required track report on activity possible enhancements to processes services provide additional supports such as helping to secure medical records.Responsibilities
Maintain communication with patients families healthcare providers at various touchpoints during patient care focusing on high-touch 24/7 patient navigation Build maintain strong working relationships with internal partners including concierge medical director dynamic schedulers practice directors physicians nurses emergency department bed management assist in exemplary patient experience for Mission Makers program participants while maintaining highest degree integrity confidentiality compliance with HIPAA Utilize technology systems to support patient navigation reporting providing regular project status updates to external stakeholders team members Track interactions share with internal stakeholders to improve processes macro micro level Successfully manages timelines resources deliverables strong program project management capabilities Demonstrate excellent teamwork communication cooperation colleagues achieve organizational objectives Assist other strategic projects duties assigned Maintain communication patients families healthcare providers at various touchpoints during their care focusing on high-touch 24/7 patient navigation Monitor satisfaction care experience work team address any issues barriers Receive respond new requests timely follow-up Provide advocacy education support accessing care Work dynamic schedulers practice managers physicians nurses multidisciplinary team achieve appointments timely manner support emergency room inpatient admission when possible Maintain communication messages EPIC emails phone calls Act single point contact referring providers patients clinical caregivers ensure coordinated efficient scheduling approach Track report daily concierge activity Must have customer-service orientation interpersonal communications organizational tracking follow through skills Act professional manner toward internal external contacts Will need reachable cell phone evenings weekends necessary Will meet patents person time plan agendas present reports schedule meetings communicate all team members perform duties assigned Requirements Bachelor degree Healthcare Hospitality related field Years nursing healthcare customer service experience travel hospitality Benefits Day One Daily Pay Career Development Tuition Reimbursement On Site Fitness Center Gottlieb Memorial Hospital LUMC Educational Stipend Certification reimbursement Referral Rewards Self SchedulingJob description
