The Intake Coordinator is responsible for the initial assessment of referrals for home health and/or hospice services and the coordination of services with the patient, physician, and referral sources.The incumbent will facilitate intake process by procuring patient information including demographics face to face encounter document clinical data prescribed home health or hospice services insurance assignment of home infusion benefits following all patient acceptance criteria policies procedures.
- Facilitates intake process by procuring patient information including but not limited to demographics face to face encounter document clinical data prescribed home health or hospice services insurance assignment of home infusion benefits following all patient acceptance criteria policies procedures.
- Collaborates with facility's discharge planning personnel to facilitate safe discharge to home health or hospice.
- Receives and processes referral information for potential patients who require home health or hospice services.
