    Description
    PURPOSE

    Provide case management and case coordination families in the Permanent Housing for Chronically Homeless (PHCH) Families at Eastman Reserves.

    Job Duties
    1. Conduct initial screenings/intake, assessments for each family referred to PHCH program.
    2. Provide case management and coordination to families residing in permanent housing in the community.
    3. Develop a working relationship with property manager.
    4. Document participants' activities, goals, and progress toward individual goal plans.
    5. Communicate and case conference regularly with supervisor, and other outside organizations working with families.
    6. Link families to appropriate services and activities within and outside the YWCA, including employment and training services, legal assistance, substance abuse programs, mental health, schools and other appropriate services. Serve as liaison between YWCA and other social agencies and community resources.
    7. Maintain accurate up-to-date records, and reports required by the funder. Monitor and follow up on residents' progress towards meeting goals.
    8. Enter accurate data into HMIS, complete quality indicators to meet community expectations for individuals living in permanent housing.
    9. Assist families in recertifying for eligible housing
    10. Case conference with property rental manager, providers, and other community resources.
    11. Participate in community workgroups/meetings as it relates to the Continuum of Care and coordinated access
    12. Demonstrates understanding of and sensitivity to cultural differences and trauma informed care.
    13. Demonstrates compliance with all appropriate HIPAA Privacy and Security Standards as defined by the YWCA of Rochester and Monroe County.
    14. Attend monthly department and all staff meeting.
    Experience and Education Qualifications
    • Associates Degree preferred in Counseling, Human Services, Sociology or Social Work. and experience with at least 2 years in case management, crisis intervention or counseling.
    • Must have experience and proficiency in database management and Microsoft Office applications.

    Additional Requirements
    • A valid and clean NYS driver's license is required. Must have a reliable vehicle and be willing to transport program participants.
