    As an RN Case Manager, your role will involve taking complete nursing responsibility in delivering the Plan of Care for each hospice patient. You will continuously assess your personal and professional performance, making necessary adjustments to enhance productivity and the quality of care provided.

    • Performing the initial nursing assessment to determine eligibility for hospice services within 48 hours of referral
    • Identifying the physical, psychosocial, and environmental needs of the patient and family, with ongoing reassessments at least every 15 days
    • Developing and coordinating the care plan, while maintaining accurate clinical records of the patient's status
    • Recording issues, setting appropriate goals, interventions, and tracking the patient/family response to hospice care
    • Collaborating with the patient/family, attending physician, and the interdisciplinary group daily to ensure comprehensive care

    Requirements:

    • Minimum of one year of clinical experience

    Licenses and Certifications:

    • Active RN license in the relevant state with at least one year of clinical experience
    • Current CPR Certification
    • Valid driver's license, reliable vehicle insurance, and access to transportation
