    Description
    Primary Care Nurse Practitioner - PACE Program

    Job Type:
    Full-time, Permanent

    Nurse Practitioner Schedule:
    Monday-Friday 8A-4:30P

    The PACE program is a center for patients 55+ in the community to come for basic healthcare needs from providers as well as have the opportunity to socialize with other patients in the area.

    Activities, games, and food are provided to members.

    The Nurse Practitioner will be responsible for providing primary care services to senior patients, including conducting assessments, diagnosing illnesses, developing treatment plans, and collaborating with the interdisciplinary team to provide comprehensive care.

    Minimum of 2 years of Primary Care experience
    Comprehensive health insurance
