    Adult Outpatient Developmental Disabilities Care Coordinator – Full-Time

    Hiring Range: $ $30.55 per hour

    Kitsap Mental Health Services (KMHS) is actively recruiting for a passionate and dedicated Developmental Disabilities Care Coordinator to join our team. As a member of our multidisciplinary team, you will play a crucial role in planning, developing, coordinating, and providing treatment, rehabilitation, and support services to our program clients.     Primary Responsibilities:

    • Serve as a liaison between Developmental Disabilities Administration (DDA) and other community resources.
    • Facilitate communication between KMHS care coordinators and DDA staff for treatment continuity.
    • Provide direct services, including individual, group, and family counseling, crisis intervention, and case management.
    • Demonstrate knowledge of the current Diagnostic and Statistical Manual of Mental Disorders.
    • Coordinate treatment with internal and external substance use treatment providers.
    • Assist clients in gaining resources and education to support their autonomy and basic needs.
    • Work collaboratively with community partners and advocacy support groups.
    • Provide crisis intervention counseling and coordinate with Designated Crisis Responders when appropriate.
    • Document client progress according to established policies and procedures.
    • Consult with families and community agencies to coordinate the treatment process.
    • Establish and maintain effective relationships with KMHS and DDA staff.

    Minimum Qualifications:

    EDUCATION: Bachelor's Degree in Psychology, social services, or behavioral health field.

    EXPERIENCE: Minimum of 1 year of job-related experience. The capacity to work well with children and families.

    This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation.

    Preferred Qualifications: In-depth knowledge of and experience with I/DD population, outstanding communication abilities, collaboration and administrative skills, and a high degree of professional insight and knowledge.
