    Saint Lukes Cancer Institute Gynecologic Oncology Nurse Navigator Job Description

    Key Responsibilities:

    1. Navigate patients through their diagnosis and treatment ensuring timely services.
    2. Meet with patient and families to provide emotional support, education and guidance to all patients.
    3. Provide medical information, education and review of reports and knowledge of community resources.
    4. Facilitate comprehensive transition of care from referring provider to first appointment.
    5. Capture and organize new patient information.
    6. Identify and address patient barriers to care.
    7. Facilitate scheduling of imaging and lab testing appointments as well as additional referrals.
    8. Communicate and educate the care process to new patients.
    9. Act as an integral part of the shared decision-making and education process for new patients.

    This position will also be required to participate in patient experience initiatives to elevate care for this patient population and stakeholders.
