Job Description:
We are looking for a motivated and experienced Clinical Pharmacist who is eager to join our team and play a significant role in patient care and the mentorship of pharmacy residents. The successful candidate will demonstrate a robust clinical pharmacy background, including Board of Pharmaceutical Specialty Certification or completion of an accredited PGY-1 or PGY-2 Residency Program and an APhA Certificate Training Program. This role offers the opportunity to lead in the accreditation and residency training processes, contributing to the growth and excellence of our pharmacy services.

Key Responsibilities:

• Foster and maintain professional relationships with all medical practice team members, customizing services to meet the unique needs of each practice.
• Provide comprehensive patient care through "Incident 2" visits, diabetes education, chronic care management, remote patient monitoring, and disease state management.
• Occasionally assist in the retail pharmacy, ensuring integrated care continuity.
• Lead the accreditation and residency training processes for our pharmacy residents.
• Collaborate with healthcare teams to develop and implement comprehensive care plans.

Requirements:

• Pharm.D. degree from an accredited college of pharmacy.
• Board of Pharmaceutical Specialty Certification or completion of an APhA/ASHP Accredited PGY-1 or PGY-2 Residency Program.
• Completion of an APhA Certificate Training Program.
• Minimum of 5 years of clinical pharmacy experience, preferably within a provider's office setting.
• Active pharmacist license in North Carolina or eligibility for licensure.
• Proven leadership capabilities in pharmacy settings, particularly in accreditation and residency training.
