    Position Summary
    Position performs community outreach, providing information on University studies and enrolling patients when appropriate. Also provides referrals to services in community.

    Job Description

    Primary Duties & Responsibilities
    • Refers patients and community members to available resources, from medical and social services to opportunities offered.
    • Assists patients with applications for Medicaid, financial assistance, and other applications (e.g., FMLA, disability, SNAP benefits, etc.).
    • Arranges language translation or interpretation services as needed.
    • Assists in arranging transportation to clinical care visits.
    • Contributes to research initiatives by working with the project team to recruit, consent, and obtain project surveys and interviews from patient and clinical team stakeholders.
    • Provides follow-up with participants by phone regarding receipt of needed clinical and social services.
    • Assists in building partnerships with community agencies.
    • Maintains proper documentation and record keeping.
    Preferred Qualifications
    • Interactive communication skills with ability to work with physicians, coordinators and patients.
    • Experience with helping patients navigate through a complex health system in regards to insurance and community resources is highly preferred.
    • Strong interpersonal and organizational skills.
    • Able to complete paperwork in timely manner with excellent accuracy.
    Required Qualifications
    • High school diploma or equivalent certification and 3 years of relevant experience in community health, human services, outreach or within a health care setting is required or bachelor's degree in Psychology, Social Work or a related field.
    • Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements.) Attainment of the Community Health Worker Certificate is required within 6 months of hire.

    Salary Range
    $ $24.66 / Hourly

    The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
