Summary:

CHRISTUS Specialty Pharmacy is hiring a full-time clinicalpharmacist with a focus on rheumatology. The pharmacist will beresponsible for the clinical management of patients withrheumatological conditions and will be working on-site in theBeaumont rheumatology clinic. The pharmacist will be responsiblefor initial and follow-up patient assessments and consultations andfor proper documentation in accordance with the pharmacy's policiesand procedures and URAC and ACHC standards. The pharmacist will beresponsible for ensuring safe and appropriate therapy andcommunicating recommended changes with medical staff. Thepharmacist will be responsible for overseeing the priorauthorization and appeals processes and offering clinicalassistance to technician staff completing the authorizations asneeded. Consults with nursing and medical staff regardingprescriptions, orders, drug and pharmaceutical detail, patientreactions, errors, and complaints. Provides prescribed medications,drugs, and other pharmaceuticals. Conducts Drug Utilization Reviewsand prepares Drug Review Criteria for medical staff review andapproval. Monitors drug therapy as assigned by medical staff.Suggests alternate drug therapy as appropriate.

• Interprets drug orders (verbal and written) andtranscribes/verifies into computerized patient medication recordaccurately.
• Maintains, accurate, completepatient drug record.
• Provides servicesefficiently and in a timely fashion.
• Maintainscompetency required for current jobtitle/position.
• Maintains current pharmacistlicense.
• Ensures applicable CE records andlicensure are maintained in department files.
• Attends staff meetings.
• Completes allcompetency/skills assessment requirements.
• Supervises and directs pharmacy supportpersonnel.
• Ensures safe, appropriate,cost-effective drug therapies for patients according to establishedpolicies, procedures, and protocols.
• Monitorsdrug therapy regimens for contraindications, drug-druginteractions, drug-food interactions, allergies and appropriatenessof drug and dose.
• Reads, extracts andinterprets information in patient medical recordsaccurately.
• Detects and reports suspectedadverse drug reactions accurately and in a timelymanner.
• Provides clinical consultation andclarification to practitioners as appropriate.
• Provides accurate, adequate and timely drug information to thehospital's professional staff.
• Provides drugeducation to patients and their families per institutionalprotocol.
• Participates in the qualityimprovement and medication use review activities of thedepartment.
• Collects data, conducts monitorsand inspections and maintains logs, records and other documentationas assigned.
• Baseline departmental standardsfor reporting interventions for clinical productivity as defined bydepartmental goals.
• Participates in antibioticstewardship, opioid stewardship, and shortage management activitiesas assigned.
• Attends interdisciplinary roundswhen assigned.
• Provides services efficientlyand in a timely fashion.

Requirements:

• • Pharm. D.required
  • Licensed in the state ofpractice
  • PGY1 residency trainingrequired
  • PGY2 ambulatory care residencytraining preferred
  • Experience in specialtypharmacy preferred
  • Experience in priorauthorization processes preferred
  • Experience inEPIC EMR preferred
