- Perform Adult/Geriatric Assessments, medical history evaluation, diagnosis, health education, physician and case management referrals, and detailed documentation following Matrix guidelines
- Collaborate with physicians, case managers, social workers, family members, and other medical personnel as needed
- Engage with Primary Care Physician (PCP) for patient education and follow-up
- Deliver services through Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree or equivalent experience with NP licensure
- Current RN and NP licensure in state of practice, including prescriptive authority or eligibility for it
- Board certification by AANP, ANCC, or AACCN in a Matrix approved specialty
- Valid BLS, ACLS, or CPR certification
- Minimum of 1 year NP experience
- Proficiency in computer skills and medical record software
- Strong verbal and written communication skills
- Adaptability to changes
- Ability to travel with a valid driver's license and adequate automobile insurance
- Travel to meet patients at their location

About the Position:
During visits that can last up to one hour, Matrix providers assess a member's health, medical history, medication adherence, social environment, and other risks. This offers valuable insights into the member's overall well-being that may be missed during standard office visits. The Matrix Comprehensive Health Assessment enhances care quality and helps address multiple care gaps efficiently.

Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $95.00 based on county per completed Assessment
Location: Home, SNF, Telehealth and other community environments in Florissant, Missouri
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits, and a Mileage Reimbursement plan
