    RN/Care Coordinator Liaison

    RN/Care Coordinator Liaison

    The Care Coordinator Liaison will work in coordination with all primary care providers including Sakakawea Medical Center, Knife River Care Center, and other community organizations to effectively coordinate and manage the care of all residents residing in a long-term care facility, home and community-based services, including visiting nurse services, palliative care, chronic care management, and remote patient monitoring.

    Experience/Requirements

    • Registered Nurse with current licensure or temporary permit from the ND Board of Nursing, required.
    • Two years of nursing experience, required.
    • Care Coordination experience preferred.
    • Excellent verbal and written communication skills required.
    • Ability to work with diverse groups of people required.
