ABOUT THE JOB
The Home Health Clinical Coordinator ensures the efficient and effective operation of the Home Health Department coordinating patient scheduling and providing direct therapy services.
Responsibilities include preparing home health visit schedules; tracking, monitoring and managing the therapy schedule of the Home Health Department; and assisting with Home Health Departmental compliance to all regulatory requirements.
