CommonSpirit has more than 700 care sites across the U.S. and is committed to building healthy communities.
Responsibilities*Job Summary:
Coordinate the most effective action to assist patients with medication and patient assistance
Essential Responsibilities:
+ Communicate timely with outside offices and providers who are requesting risk stratification for our mutual patients.+ Triage patient appropriately per periprocedural cardiac risk evaluation algorithm.+ Review and complete appropriate patient documentation for periprocedural cardiac risk evaluation following algorithm approved by physician group.+ Coordinate as needed with clinical staff member assigned to Periprocedural Antithrombotic Management form.+ Assist PA/FMLA RN when assigned by manager.
Qualifications
Education:
+ Graduate from an accredited RN program with one year in a clinical nursing setting
License/Certification:
+ Valid RN license in the state of Tennessee+ BLS Certification
Qualifications:+ Knowledge of EHR systems.+ Excellent written and verbal communication skills.+ Cardiology experience a plus.
