- Conducts the initial and/or comprehensive nursing evaluation visit, identifies the primary care focus, formulates the care plan following state-specific directives, and submits precise, thorough, and punctual documentation in coordination with physician prescriptions.
- Carries out comprehensive patient assessments at specified intervals and devises the suitable care plan.
- Confirms that patients fulfill the home health eligibility criteria and medical necessity standards as outlined by the payer source.
- Active RN authorization in the state of practice
- Current CPR certification
- Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transit
- AL: No other state-specific conditions apply.
