    **Join Our Dynamic Team as an LMSW**

    **Job Description:**
    - **Position:** Licensed Masters of Social Worker (LMSW)
    - **Shift:** Day 5x8-Hour (07:30 - 16:00)
    - **Experience Required:** 1 year
    - **Patient Ratio Experience:** Varied (1:0 on Days)
    - **Charting System:** Epic (experience required)
    - **Travel Experience:** Not required
    - **Certifications:**
    - MSW (required)
    - BLS (required)
    - **Skills Preferred:**
    - Acute Hospital Experience
    - Emergency Department, ICU, L&D/Mother-Baby, NICU
    - Knowledge of Advance Directives, ADA, high-risk patient vulnerabilities, cultural issues, and communication barriers.

    **Work Settings:**
    - Short Term Acute Care Inpatient Medical Social Worker

    **Assessment Duties:**
    - Conduct comprehensive biopsychosocial assessments
    - Perform initial evaluations

    **Care Coordination Responsibilities:**
    - Collaborate effectively in care teams
    - Coordinate home care and necessary equipment post-discharge
    - Ensure quality care is delivered in a cost-effective manner
    - Facilitate referrals to rehabilitation and long-term care facilities

    **Documentation/Communication:**
    - Maintain thorough documentation of care delivery, plans, and goals
    - Knowledge of community programs including Medicaid, Medicare, and transport services
    - Understand protective services such as adult protective services and guardianship

    **Case Management/Utilization Review:**
    - Engage in care coordination and discharge planning
    - Conduct needs assessments and plan of care

    **Additional Details:**
    - **Float Requirements:** Yes, flexibility to work in various hospital areas
    - **Weekend Coverage:** Required every other weekend
    - **Orientation Hours:** 40
    - **Patient Age Groups:** Adults, Geriatrics
    - **Daily Census:** 25 patients, weekends up to 30 patients
    - **Interdisciplinary Support Team:** Includes Physical Therapy, Interpretation Services, Social Services, Pharmacy, and Transportation
    - **Facility Type:** Level II Trauma Center, with a total of 571 staffed beds across two campuses
