    Job Summary: Under the supervision and in collaboration with a licensed physician, who is a member of the active medical staff, performs various medical duties for the hospital and in participating in patient management as a member of a treating team within the scope of practice of an APP. Provides team direction on assigned unit as necessary. Reports to the designated Division Medical Director of appropriate service.

    PROVIDES GENERAL CARE AND TREATMENT OF PATIENTS WITHIN THE SCOPE OF PRACTICE OF AN APP.

    • Undertakes only those clinical procedures for which delineated privileges have been requested, clinical competence has been demonstrated, and which have been approved by the Board of Directors of Gaylord Hospital.
    • Obtains medical history and performs physical examinations on patients in collaboration with the physician and other members of the treating team.
    • Prescribes a diagnostic and treatment plan on admission including but not limited to laboratory, tests, x-rays, consults, and referrals.
    • Documents information appropriately and according to standards set in Medical Staff Bylaws, Rules and Regulations, and policies.
    • Prescribes medications in accordance with state and federal regulations
    • Evaluates physiological and behavioral responses to interventions, discusses changes in patients' status with the attending physician and other clinical staff, and prescribes alterations in the treatment plan as needed.
    • Incorporates research data into practice as appropriate.
    • Responds to urgent problems in the hospital, as appropriate.
    • Becomes actively involved in various administrative committees and projects as determined by the CMO or designee. Also participates in quality improvement projects.
    • When necessary, performs procedures and other tests within prescribed limits under the general supervision of a member of the medical staff.
    • Becomes an integral part of the team and is willing to provide backup as needed.

    PARTICIPATES AS A MEMBER OF THE INTERDISCIPLINARY TEAM

    • Attends and participates in unit team meetings in collaboration with the attending physician.
    • Participates in family conferences and treating team conferences.
    • Provides patients and their families with relevant and understandable information and recommendations concerning their illnesses and ongoing treatment.
    • Collaborates with team members to develop and implement a coordinated comprehensive therapeutic plan for each patient.
    • Observes patients in therapy department as needed.
    • Collaborates with the patient, family, and other members of the team in developing discharge plans and services as needed post-discharge.

    PROVIDES EDUCATION FOR PATIENT, FAMILIES AND STAFF AS APPROPRIATE.

    • Participates in patient and family educational activities including family and treating team conferences.
    • Provides patients and their families with relevant and understandable information and recommendations concerning their illness and ongoing treatment.
    • Participates in educational programs for staff.
    • Serves as a resource to staff and students.

    ADMINISTRATIVE/APP

    • Participates on medical staff committees.
    • Participates on hospital committees as requested.
    • Assists in patient unit management as needed.
    • Attends hospital meetings as required.
    • Participates in FPPE and OPPE as required.
    • Provides clinical consultation
    • Collaborates with Medical Director in formulating protocols as needed
    • Participates in the orientation and training of new staff physicians, APP's, students, and other staff as appropriate.

    Qualifications:

    • Must have excellent general medical clinical skills.
    • Ability to provide inpatient and/or outpatient care to adolescent through geriatric patients.
    • Communication and interpersonal skills
    • Clinical assessment and evaluation skills.
    • Knowledge of relevant state and federal laws.
    • Adult, inpatient medical/surgical or rehabilitation experience.
    • One year of post-training program experience preferred, but not required
    • Must be a graduate of an accredited APP program
    • Current license as an Advanced Practice Provider issued by the Connecticut Department of Public Health and Addiction Services
    • Maintains active certification as an APP
