- The social work supervisor provides direct patient care, including but not limited to: psychosocial assessment, care planning, mandated reporting, and concrete resource needs.
- The supervisor provides crisis intervention and care coordination to patients and families as necessary to meet immediate psychosocial needs.

- The supervisor assists with the creation of SBARs and ProForma in relation to initiatives as assigned.
- The supervisor collaborates with other members of the Care Management team to create interdisciplinary programming that optimizes patient and family health. The supervisor works with their social work team to understand resources to network with other community resources, thereby optimizing program opportunities as much as possible.
- The supervisor makes decisions regarding use of department financial resources appropriately.

The supervisor arranges for new staff orientation and ensures that all existing staff are current on required staff education and annual competencies. The supervisor conducts all performance management activities including coaching and progressive discipline.
- The supervisor arranges for and facilitates team social work staff meetings. The supervisor provides ongoing guidance and supervision of assigned social workers, giving them both ongoing encouragement and constructive feedback as required. The supervisor conducts periodic formal staff assessments and annual peformance reviews. The supervisor is expected to be knowledgeable about and to follow AFSCME contracts and associated work rules. The supervisor advocates for Social Work staff to participate in professional development at a local, regional, and national level.

As a member of the Care Management Social Work Leadership team, the supervisor provides strategic leadership and direction for Social Work Programs.
They keep abreast of trends in psychosocial theories, and practice frameworks to understand the impact and adjustment to illness, clinical interventions to encourage and strengthen health coping and adjustment, resources to enhance patient and family well-being, transitional care, illness and wellbeing across the lifespan, patient and family-family centered practices, end of life and palliative care considerations, psychoeducational support and mentoring models, interdisciplinary collaboration, mental and behavioral health, chemical dependency, interpersonal violence, homelessness, community resources, care coordination, and trauma-informed care.
The supervisor provides supervision to all assigned social workers and participates in on-call responsibilities.They must hold competency across clinical areas (ED, inpatient, ambulatory, transitional care, pediatrics, and adults) in order to respond to the diverse needs of the organization.
They are responsible for recruitment, hiring, retention, orientation, scheduling, and professional development of all assigned social workers. The supervisor participates in on-call responsibilities. The supervisor also provides ongoing direct care for patients served when required.Salary Range:
$81,619 to $130,374
